For patients recently discharged from a hospital, SNF, inpatient rehab, or other qualifying facility. Provides intensive 30-day post-discharge support.
Chronic Care Management
For patients with two or more chronic conditions expected to last 12+ months. Provides ongoing care coordination and responsive care team access.
