Lowryanne founded Proactive Care Partners to address a critical gap in post-discharge and chronic disease care. With advanced clinical training and a passion for proactive intervention, she built a practice dedicated to keeping patients healthy, informed, and out of the hospital — one day at a time.
Our Mission
Proactive Care Partners was founded by Lowryanne Vick, DNP, APRN, ACNP-BC on a simple but powerful idea: that patients with complex health needs deserve more than a 15-minute office visit every few months. The gaps between appointments — the days after a hospital discharge, the weeks between specialist visits, the moments when a medication question goes unanswered — are where outcomes are won or lost.
Our mission is to close those gaps. Through our Transitional and Chronic Care Management programs, we provide the continuous, coordinated care that keeps patients healthy, informed, and out of the hospital. Focused care. Early intervention. Stronger recovery. Fewer readmissions.

Meet Our Founder
Lowryanne Vick, DNP, APRN, ACNP-BC
Founder & Provider
Dr. Lowryanne Vick, Founder and Provider of Proactive Care Partners, created the practice to bridge the gap between hospital discharge and long-term wellness. Throughout her career, she witnessed too many patients leave the hospital only to struggle with medication changes, delayed follow-up appointments, unanswered questions, and preventable complications. She created Proactive Care Partners to provide the support patients need during this critical transition.
As a board-certified Adult Acute Care Nurse Practitioner with more than three decades of healthcare experience, Dr. Vick specializes in caring for adults with complex acute and chronic medical conditions. Her practice combines evidence-based medicine with a personalized approach that includes home visits, telehealth, Remote Patient Monitoring (RPM), Transitional Care Management (TCM), and Chronic Care Management (CCM).
In addition to caring for patients, Dr. Vick has served in national leadership roles advancing telehealth, digital health, and remote patient monitoring. She has designed educational programs, led implementation initiatives, and collaborated with healthcare professionals across the country to improve access to care through innovative technology. These experiences have shaped her vision for delivering healthcare that is both compassionate and forward-thinking.
Dr. Vick earned her Doctor of Nursing Practice degree from the University of Illinois Chicago and her Master of Science in Nursing as an Adult Acute Care Nurse Practitioner and Critical Care Clinical Nurse Specialist from Rush University. She is committed to lifelong learning and applying the latest evidence-based practices to help patients achieve the best possible outcomes.
